چکیده

Objectives: To determine the time to surgery (TTS) among patients with acute hip fracture who are anticoagulated vitamin K antagonists (VKAs) receive prothrombic complex concentrate (PCC) compared those do not PCC.Method: We conducted a retrospective cohort study of consecutive an presenting emergency department (ED) Jewish General Hospital, 637-bed tertiary care hospital center in Montreal, Quebec, between January 1st, 2015 and March 2020. Eligible were identified via ED electronic database. Inclusion criteria were: (1) > 18 years age; (2) admission for surgical repair; (3) VKA use international normalized ratio (INR) 1.5 at admission. Follow-up was limited index hospitalization. The primary outcome TTS, reported hours, defined as from surgery. Mann-Whitney U test used assess any difference median TTS two groups. Results: A total 53 included our which 43.4% (23/53) received PCC first dose 25.5 hours (IQR 19.4-51.6). 84.9 % (45/53) 16 h 7.5-26.7). no groups 51.8 26.8–71.4) vs. 63.5 49.4-73.2) respectively (p=0.71). Conclusion: This did identify shorter not. Median both surpassed recommended 48h benchmark, administration timing suboptimal. Safety of prothrombin complex concentrates for rapid anticoagulation reversal of vitamin K antagonists. A meta-analysis.

Prothrombin complex concentrates (PCCs) are recommended as the treatment of choice in warfarin-related coagulopathy. However, the risk of thromboembolic complications associated with their use is not well defined. We performed a meta-analysis to estimate the rate of thromboembolic complications in patients receiving vitamin K antagonists (VKAs) treated with PCCs for bleeding or before urgent su...

Prothrombin complex concentrates for reversal of vitamin K antagonists: assessing the risks.

Vitamin K antagonists (VKA) have been the standard for the treatment and prevention of thromboembolism in patients with atrial fibrillation, venous thromboembolism and prosthetic heart valves for more than 50 years. In 2004, more than 30 million prescriptions for warfarin were written in the United States (1). VKA are subject to significant dietand drug-drug interactions; therefore, it is not s...

Treatment with vitamin K in hip fracture patients receiving warfarin.

BACKGROUND Hip fractures are common in the elderly population, but surgical treatment of these fractures within the first 48 hours decreases morbidity and mortality. The management of patients with hip fracture requiring surgery who are taking warfarin anticoagulation is unclear. OBJECTIVES To determine the effect of vitamin K on hip fracture patients treated with warfarin. METHODS We retro...

Feasibility and clinical effectiveness of pre-hospital prothrombin complex concentrates (Octaplex/Beriplex) in anticoagulated patients with traumatic intracranial haemorrhage

Background Incidence and mortality from traumatic intra-cranial haemorrhage (ICH) is increased in patients on warfarin [1]. Prothrombin Complex Concentrates (PCC) rapidly reverse warfarin anti-coagulation and have been shown to reduce haematoma expansion and mortality in warfarinised patients sustaining ICH [2]. We aimed to determine the feasibility of pre-hospital use of PCCs by physicians and...
